Volleyball Agreement
1. PURPOSE OF THIS AGREEMENT
Participation on the New Jerusalem Academy Girls Volleyball Team is a privilege and an extracurricular athletic opportunity intended to promote physical fitness, teamwork, discipline, leadership, perseverance, responsibility, sportsmanship, and positive character development.
By signing this agreement, the parent/guardian gives permission for the student named above to participate in volleyball tryouts, practices, conditioning, games, tournaments, team events, and other volleyball-related activities sponsored, supervised, or approved by New Jerusalem Academy.
I understand that participation in athletics requires physical exertion and carries inherent risks of injury, even when appropriate safety precautions, instruction, equipment, and supervision are provided.
2. ACKNOWLEDGMENT OF RISKS
I understand that participation in volleyball may involve risks, including but not limited to:
Sprains, strains, and muscle injuries;
Bruises, scrapes, cuts, and abrasions;
Finger, hand, wrist, arm, shoulder, knee, ankle, foot, and other joint injuries;
Injuries resulting from jumping, landing, running, stopping, or changing direction;
Collisions with teammates, opponents, coaches, officials, walls, poles, nets, benches, or other equipment;
Falls during play or practice;
Overuse injuries and muscle soreness;
Concussions and other head injuries;
Heat-related illness, dehydration, or exhaustion;
Injuries related to playing surfaces or athletic equipment;
Accidental contact with another participant;
Other injuries, illnesses, or medical conditions associated with athletic participation.
I understand that certain risks are inherent in the sport and cannot be completely eliminated, even with proper supervision and reasonable safety precautions.
3. VOLUNTARY PARTICIPATION
I understand that my daughter's participation in the NJA Girls Volleyball Team is voluntary.
I have discussed the nature and potential risks of participation with my daughter and believe she is physically and emotionally capable of participating.
I agree that my daughter will follow all instructions provided by NJA coaches, teachers, administrators, athletic personnel, officials, and other authorized adults.
4. MEDICAL INFORMATION
I certify that I have provided NJA with accurate and complete information regarding my daughter's medical history, including any known:
Medical conditions;
Allergies;
Injuries;
Physical limitations;
Medications;
Dietary or other relevant health concerns.
I agree to promptly notify NJA if there is any change in my daughter's health or physical condition that could affect her ability to participate safely.
5. FIRST AID AND EMERGENCY MEDICAL CARE
In the event of an injury, illness, or medical emergency, I authorize NJA personnel to provide reasonable first aid and to seek emergency medical assistance when they determine that such assistance is necessary.
I authorize NJA personnel to contact me or my designated emergency contact as soon as reasonably possible.
If emergency medical treatment is necessary and I cannot be reached in a timely manner, I authorize appropriate emergency personnel to provide treatment consistent with applicable medical standards.
I understand that NJA personnel are not medical professionals and that emergency situations may require immediate action before a parent/guardian can be reached.
6. MEDICAL AND OTHER EXPENSES
I understand that NJA does not assume financial responsibility for medical, hospital, ambulance, physician, dental, rehabilitation, prescription, or other expenses arising from my daughter's participation in volleyball, except to the extent required by applicable law.
I understand that the parent/guardian is responsible for maintaining appropriate health and accident insurance coverage for the student.
7. INJURY AND RETURN TO PARTICIPATION
I understand that NJA personnel may remove my daughter from a practice, game, or other athletic activity if she is injured, becomes ill, or demonstrates symptoms that raise a safety or medical concern.
I agree that my daughter will not continue participating against the direction of NJA personnel when continued participation could reasonably place her or another participant at risk.
When appropriate, NJA may require medical evaluation or written clearance from a qualified healthcare provider before allowing the student to return to athletic participation.
8. ATHLETIC CONDUCT AND SPORTSMANSHIP
I understand that my daughter is expected to:
Treat teammates, opponents, coaches, officials, teachers, and other adults respectfully;
Demonstrate good sportsmanship;
Follow all NJA school policies and team rules;
Follow directions given by coaches and NJA personnel;
Use athletic equipment appropriately;
Maintain appropriate language and behavior;
Refrain from fighting, bullying, harassment, intimidation, threats, or physical aggression;
Refrain from intentionally or recklessly engaging in conduct that could harm another person;
Demonstrate self-control and respect, both on and off the court.
I understand that violations of NJA policies or team expectations may result in disciplinary action, loss of playing time, suspension, or removal from the team.
9. ACADEMIC RESPONSIBILITIES
I understand that student-athletes are expected to maintain their academic responsibilities while participating in athletics.
Participation may be limited or suspended if a student is not meeting NJA's academic, attendance, behavioral, or other applicable school expectations.
Participation on the volleyball team does not excuse missed assignments, excessive absences, tardiness, or failure to fulfill academic responsibilities.
10. ATTENDANCE AND TEAM COMMITMENT
I understand that volleyball is a team sport and that consistent attendance at practices, games, tournaments, and other team activities is important.
I agree to notify the coach or NJA when my daughter is unable to attend a scheduled activity.
Repeated unexcused absences, tardiness, or failure to demonstrate appropriate commitment to the team may affect playing time or continued participation.
11. APPROPRIATE ATTIRE AND EQUIPMENT
I understand that my daughter is expected to wear appropriate athletic clothing, footwear, protective equipment, and any required NJA team uniform or equipment.
I understand that proper athletic shoes and appropriate attire are important for safety and performance.
My daughter agrees to properly use and care for any NJA-issued or team equipment.
12. PERSONAL PROPERTY
I understand that NJA is not responsible for personal belongings that are lost, stolen, or damaged during practices, games, tournaments, or other team activities, except as required by applicable law.
Students are encouraged not to bring unnecessary valuables to athletic activities.
13. TRANSPORTATION
When transportation is provided or arranged by NJA for an approved volleyball activity, I authorize my daughter to travel using transportation arranged or approved by NJA.
Transportation may include school-authorized buses, vans, vehicles, or other approved transportation arrangements.
When NJA does not provide transportation, the parent/guardian is responsible for arranging transportation to and from the activity unless otherwise communicated by the school.
I understand that students must follow all transportation rules and instructions provided by NJA personnel.
14. PARENT/GUARDIAN CONDUCT
I understand that parents and guardians are expected to support the team in a respectful and positive manner.
Parents/guardians are expected to:
Demonstrate good sportsmanship;
Respect coaches, officials, opposing teams, students, and other families;
Refrain from inappropriate, threatening, or disruptive behavior;
Direct questions or concerns through the appropriate NJA communication process;
Avoid interfering with coaching decisions or game officials during athletic activities.
NJA reserves the right to address inappropriate parent/guardian conduct in accordance with school policies.
15. RELEASE AND WAIVER
To the fullest extent permitted by Florida law, I voluntarily agree to release and hold harmless New Jerusalem Academy, its governing organization, administrators, employees, coaches, teachers, volunteers, and authorized representatives from claims arising from my daughter's participation in NJA volleyball activities, including claims relating to ordinary negligence, except to the extent such release is prohibited by applicable law or arises from conduct that cannot legally be waived.
This provision is intended to apply only to risks and circumstances legally capable of being released or waived.
16. PHOTOGRAPHY, VIDEO, AND MEDIA
I understand that NJA may photograph or record students during school-approved athletic activities for educational, athletic, yearbook, school communication, website, or promotional purposes.
17. COMMUNICATION
I understand that NJA may communicate with parents/guardians regarding practices, games, schedules, transportation, changes, team expectations, and other volleyball-related matters through school-approved communication methods.
I agree to maintain current contact information with the school and to promptly review communications regarding my daughter's participation.
18. PARENT/GUARDIAN ACKNOWLEDGMENT
By signing below, I acknowledge and agree that:
I have read this agreement carefully. _____
I understand the nature of volleyball participation and the inherent risks associated with the sport. _____
I have had the opportunity to ask questions regarding this agreement. ______
I have provided accurate medical and emergency contact information. _____
I authorize my daughter to participate in the NJA Girls Volleyball Team. ______
I understand that my daughter must follow NJA policies, team rules, coach instructions, and safety requirements. _____
I understand that athletic participation is a privilege and may be limited, suspended, or revoked for legitimate academic, behavioral, attendance, safety, or disciplinary reasons.
I understand that this agreement does not waive any rights that cannot legally be waived under Florida law. ______
I agree to support my daughter in maintaining appropriate conduct, sportsmanship, academic responsibility, and commitment to the team. ________
19. STUDENT PICKUP, SIBLINGS & AFTER-CARE
Parents/guardians are responsible for arranging timely pickup and supervision of any siblings or other students who are not participating in the scheduled athletic activity.
Non-participating students may not remain on campus after regular school hours simply because a sibling is attending a practice, game, or other athletic activity. Parents/guardians must pick up these students by 5:00 PM unless the student has been approved to participate in an NJA after-care program or another school-authorized activity.
NJA does not provide supervision for students remaining on campus after 5:00 PM outside an authorized school program and, to the fullest extent permitted by law, is not responsible for students left on campus without authorized supervision.
If after-care is needed for a sibling or other student, please contact the NJA Administration to discuss available arrangements.
20. ATHLETIC FEES & FINANCIAL RESPONSIBILITY
I understand that participation in NJA Athletics may include additional costs such as team fees, uniforms, equipment, tournaments, games, transportation, or other approved athletic expenses.
As the parent/guardian of a student-athlete, I agree to financially support my student's participation by paying applicable athletic fees and expenses communicated in advance by the coaches or NJA Administration. I understand that meeting these financial commitments is part of my student's participation in the athletic program.
