Basketball Agreement

1. PURPOSE OF THIS AGREEMENT

Participation on the New Jerusalem Academy Basketball Team is a privilege and an extracurricular athletic opportunity designed to encourage physical fitness, teamwork, discipline, perseverance, leadership, sportsmanship, responsibility, and character development.

By signing this agreement, the parent/guardian gives permission for the student named above to participate in basketball tryouts, practices, conditioning, games, tournaments, team activities, and other basketball-related activities sponsored, supervised, or approved by NJA.

I understand that basketball involves physical activity and inherent risks of injury, even when reasonable safety measures and appropriate supervision are provided.

2. ACKNOWLEDGMENT OF RISKS

I understand that basketball participation may involve risks, including but not limited to:

  • Sprains, strains, and muscle injuries;

  • Bruises, scrapes, cuts, and abrasions;

  • Finger, hand, wrist, arm, shoulder, knee, ankle, foot, and joint injuries;

  • Collisions with teammates, opponents, coaches, officials, walls, bleachers, basketball goals, benches, or other objects;

  • Falls resulting from running, jumping, pivoting, stopping, or changing direction;

  • Contact-related injuries;

  • Overuse injuries and muscle soreness;

  • Concussions and other head injuries;

  • Heat-related illness, dehydration, or exhaustion;

  • Injuries resulting from playing surfaces, equipment, or other conditions;

  • Other injuries or medical conditions associated with athletic participation.

I understand that the physical nature of basketball means that certain risks cannot be completely eliminated.

3. VOLUNTARY PARTICIPATION

I understand that my child's participation in the basketball program is voluntary.

I have discussed the nature and potential risks of basketball participation with my child and believe that my child is physically and emotionally capable of participating.

I agree that my child will follow all instructions provided by NJA coaches, teachers, administrators, athletic personnel, officials, and other authorized adults.

4. MEDICAL INFORMATION AND EMERGENCY CARE

I certify that I have informed NJA of any known medical conditions, allergies, injuries, medications, physical limitations, or other circumstances that may affect my child's ability to safely participate.

I agree to notify NJA promptly of any changes in my child's health or any injury or illness that may affect participation.

In the event of an injury or medical emergency, I authorize NJA personnel to provide reasonable first aid and to seek emergency medical assistance when deemed necessary.

I authorize NJA personnel to contact me or my designated emergency contact as soon as reasonably possible.

5. MEDICAL EXPENSES

I understand that NJA does not assume financial responsibility for medical, hospital, ambulance, physician, dental, rehabilitation, prescription, or other expenses arising from my child's participation in basketball, except as otherwise required by applicable law.

I understand that it is my responsibility to maintain appropriate health and accident insurance coverage for my child.

6. INJURY AND RETURN TO PLAY

I understand that NJA personnel may remove my child from participation if my child is injured, becomes ill, or demonstrates symptoms that raise a safety or medical concern.

My child will not be permitted to continue participating when NJA personnel determine that continued participation could present a safety concern.

When appropriate, NJA may require medical evaluation or documentation from a qualified healthcare provider before the student returns to athletic participation.

7. RULES, CONDUCT, AND SPORTSMANSHIP

I understand that my child is expected to:

  • Treat teammates, opponents, coaches, officials, teachers, and other adults with respect;

  • Demonstrate good sportsmanship;

  • Follow all team and school rules;

  • Follow coach and NJA staff instructions;

  • Use basketball equipment appropriately and safely;

  • Maintain appropriate language and behavior;

  • Refrain from fighting, bullying, harassment, intimidation, threats, or physical aggression;

  • Refrain from intentionally or recklessly pushing, striking, tripping, or otherwise physically harming another person;

  • Demonstrate self-control and respect during practices and games.

I understand that violations of NJA policies or team expectations may result in consequences including loss of playing time, suspension, or removal from the team.

8. ACADEMIC RESPONSIBILITIES

I understand that student-athletes remain students first and are expected to maintain their academic responsibilities.

Participation may be limited or suspended if a student fails to meet NJA's academic, attendance, behavioral, or other applicable expectations.

Being a member of an athletic team does not excuse missed assignments, excessive absences, tardiness, or failure to meet academic responsibilities.

9. ATTENDANCE AND TEAM COMMITMENT

I understand that basketball is a team sport and that consistent attendance is important to the success of the team.

I agree to notify the coach or NJA when my child will be absent from a scheduled practice, game, tournament, or team activity.

Repeated unexcused absences or tardiness may affect playing time or continued participation.

10. EQUIPMENT AND PERSONAL PROPERTY

I understand that my child is responsible for properly using and caring for NJA-issued or team equipment.

I understand that NJA is not responsible for personal belongings that are lost, stolen, or damaged during practices, games, tournaments, or team activities, except as required by applicable law.

11. TRANSPORTATION

When transportation is provided or arranged by NJA for an approved basketball activity, I authorize my child to travel using transportation arranged or approved by NJA.

Transportation may include school-authorized buses, vans, vehicles, or other approved transportation arrangements.

When NJA does not provide transportation, the parent/guardian is responsible for transportation to and from the activity unless otherwise communicated by the school.

12. RELEASE AND WAIVER

To the fullest extent permitted by Florida law, I voluntarily agree to release and hold harmless New Jerusalem Academy, its governing organization, administrators, employees, coaches, teachers, volunteers, and authorized representatives from claims arising from my child's participation in basketball activities, including claims relating to ordinary negligence, except to the extent such release is prohibited by applicable law or arises from conduct that cannot legally be waived.

This provision is intended to apply only to risks and circumstances legally capable of being released or waived.

13. PHOTOGRAPHY AND MEDIA

I understand that NJA may photograph or record students during school-approved athletic activities for educational, athletic, school communication, yearbook, website, or promotional purposes.

14. PARENT/GUARDIAN ACKNOWLEDGMENT

By signing below, I acknowledge that:

  1. I have read this agreement carefully.

  2. I understand the nature of basketball participation and its inherent risks.

  3. I have had the opportunity to ask questions regarding this agreement.

  4. I have provided accurate emergency and medical information.

  5. I authorize my child to participate in NJA basketball activities.

  6. I agree that my child must follow NJA policies, athletic rules, and coach instructions.

  7. I understand that athletic participation is a privilege and may be revoked for legitimate academic, behavioral, attendance, safety, or disciplinary reasons.

  8. I understand that this agreement does not waive any rights that cannot legally be waived under Florida law.

15. STUDENT PICKUP, SIBLINGS & AFTER-CARE

Parents/guardians are responsible for arranging timely pickup and supervision of any siblings or other students who are not participating in the scheduled athletic activity.

Non-participating students may not remain on campus after regular school hours simply because a sibling is attending a practice, game, or other athletic activity. Parents/guardians must pick up these students by 5:00 PM unless the student has been approved to participate in an NJA after-care program or another school-authorized activity.

NJA does not provide supervision for students remaining on campus after 5:00 PM outside an authorized school program and, to the fullest extent permitted by law, is not responsible for students left on campus without authorized supervision.

If after-care is needed for a sibling or other student, please contact the NJA Administration to discuss available arrangements.

16. ATHLETIC FEES & FINANCIAL RESPONSIBILITY

I understand that participation in NJA Athletics may include additional costs such as team fees, uniforms, equipment, tournaments, games, transportation, or other approved athletic expenses.

As the parent/guardian of a student-athlete, I agree to financially support my student's participation by paying applicable athletic fees and expenses communicated in advance by the coaches or NJA Administration. I understand that meeting these financial commitments is part of my student's participation in the athletic program.
